STEP 1 - OHSAA Physical Examination Form
It is required by state law that each student athlete have a current OHSAA Physical on file BEFORE they can participate. Please turn in the completed physical to Athletic Director Trey Jones or our Head Athletic Trainer Pam Stotts.
Physicals are valid for 13 months.

STEP 2 - Final Forms
Each athlete AND parent must have a Final Forms account and have ALL forms signed before they can participate. All forms must be updated before each season. Make sure to select ALL sports your student athlete is interested in participating in. Create and manage your accounts at the link below.

STEP 3 - PaySchools Central
Dublin City Schools Middle School Athletics require a $30 Pay to Participate Fee for each sport. The preferred method of payment is through PaySchools Central. Click below to Pay your Fee. If you are trying out for a cut sport, (Volleyball, Cheer, Tennis, Basketball or Baseball) please wait until your student athlete has made the final roster to pay this fee.
